The paradox symptom — watery eyes: Many dry eye patients present with excessive watering. This occurs because the unstable tear film triggers a reflex watering response — but reflex tears lack the lipid component needed to stabilise the tear film, so the eye remains dry despite the watering.

How Dry Eye is Diagnosed

A proper tear film assessment is essential before treatment is prescribed. Guessing the type and prescribing the wrong drops is one of the most common causes of treatment failure. At BEH, my assessment includes:

Tear Film Tests:

  • TBUT (Tear Break-Up Time) — normal >10 seconds; <5 seconds indicates significant instability

Best Treatment for Dry Eye in Dhaka — What Actually Works

Treatment must be matched to the type and severity of dry eye. There is no single drop that works for everyone.

Step 1 — Artificial Tears and Lubricating Drops

  • Carboxymethylcellulose (CMC) drops — e.g. Refresh, Tears Naturale. Good for aqueous-deficient dry eye. Widely available in Bangladesh

Dry Eye and Vision Correction Surgery — What Every Patient Must Know

LASIK can worsen pre-existing dry eye. The corneal flap severs nerves driving lacrimal secretion — post-LASIK dry eye affects an estimated 20–40% of patients in the first 3–6 months and can persist longer in those with pre-existing disease.

For patients with significant dry eye considering vision correction, I recommend SMILE or ICL depending on their eye profile — both carry substantially lower dry eye risk than LASIK. Color vision deficiency (CVD) is the clinically precise term. “Color blindness” is the common name — and it is slightly misleading, because the vast majority of people with CVD are not completely blind to color. They see color — just less of it, or less accurately than people with normal trichromatic vision.

This is the section I want to be most careful about — because there is a great deal of marketing language around color blind lenses that can give patients unrealistic expectations.

Here is the honest mechanism:

The most common form of red-green color blindness involves an excessive overlap between the sensitivity curves of the red and green cone photoreceptors.   • Patients with achromatopsia — complete absence of cone function; lenses cannot create cone sensitivity that does not exist

This is a dimension of color vision deficiency that rarely gets discussed in medical articles — but it matters enormously to my patients in Bangladesh.

Many professions in Bangladesh require passing a formal color vision test as a condition of employment or licensing. These include:

  • Bangladesh Navy, Army, and Air Force — color vision standards strictly enforced for active duty roles
  • Civil Aviation — pilots and air traffic controllers require full color vision or correctable CVD
  • Bangladesh Police Service — color vision testing at recruitment

For patients in these categories, the goal is not just seeing more colors — it is passing the Ishihara or Farnsworth D-15 test that stands between them and their career. 3. Farnsworth D-15 Panel Test

4. Rabin Cone Contrast Test (RCCT)
A more sensitive, computerised test that quantifies the specific degree of red, green, and blue cone sensitivity. Particularly relevant for professional color vision testing.

5. Anomaloscopy
The gold standard for precisely classifying the type and severity of red-green deficiency — distinguishing anomalous trichromacy from dichromacy with precision. Guides the exact wavelength filter prescription.

6. Full Eye Examination
It is important to rule out acquired causes of color vision change — cataracts, macular disease, glaucoma, optic neuritis, or medication side effects — particularly in adults whose color vision has changed over time. Color vision changes that come on later in life can point to eye disease, cataracts, neurologic problems, or medication side effects, and those need a clinical workup rather than a tinted lens.

Can Color Blindness Be Permanently Cured?

I want to answer this directly, because patients often come to me having read about gene therapy and hope it may be available in Bangladesh now.

To date, there is no therapeutic solution that can restore typical color vision. There have been considerable developments concerning technological interventions — hardware-based aids including tinted glasses and filters, and software-based methods for enhancing colour perception in digital environments.

Gene therapy for color blindness has shown promising early results in animal studies — and limited human trials have begun in the US for certain rare forms of achromatopsia. However, as of 2026, gene therapy for common red-green color blindness is not clinically available anywhere in the world, including Bangladesh. It remains a research-stage intervention.

What is available and proven:

  • Color blind spectacle and contact lenses — practical, accessible, beneficial for the majority of red-green CVD patients
  • Digital tools and apps — smartphone apps that identify and name colors, colorblind-friendly screen modes built into modern devices
  • Environmental adaptation strategies — labelling systems, colour-coded organisation tools

What is not yet available:

  • A permanent medical cure for inherited color vision deficiency
  • Gene therapy outside of clinical trial settings
  • Any eye drop, supplement, or exercise that restores cone function

WHO Guidelines for Management of Corneal Ulcers — What Every Patient Should Know

A corneal ulcer is one of the most serious eye emergencies I encounter in clinical practice. In my years at Bangladesh Eye Hospital, I have seen patients arrive with corneas that were near-perforation — not because their condition was untreatable, but because they did not know the warning signs, did not seek care quickly enough, or received incorrect initial treatment at the primary level.

The World Health Organization (WHO) published guidelines specifically for the South-East Asia region on the management of corneal ulcers across primary, secondary, and tertiary care facilities. As a cornea-trained ophthalmologist who completed my fellowship at LV Prasad Eye Institute — one of Asia’s leading corneal centres — I want to translate these guidelines into clear, practical information that patients and healthcare workers in Bangladesh can actually use.

This article covers what a corneal ulcer is, why it is an emergency, how it should be managed at each level of care, and when urgent referral to a cornea specialist is essential.


What is a Corneal Ulcer?

The cornea is the clear, dome-shaped front surface of the eye — the transparent window through which light enters. It plays a critical role in focusing vision and acts as the eye’s primary physical barrier against infection and injury.

A corneal ulcer is an open wound or erosion of the corneal tissue, typically caused by infection. It is not a superficial scratch. A corneal ulcer penetrates into the layers of the cornea itself — and if left untreated or poorly treated, it can destroy the cornea, cause perforation of the eye, and result in permanent vision loss or loss of the eye entirely.

Infectious keratitis, often manifesting as a corneal ulcer, is a significant cause of ocular morbidity worldwide — ranking among the top five causes of blindness globally, and disproportionately impacting marginalized groups. Dr. Agarwals

In Bangladesh, corneal ulcers are particularly common due to agricultural work (plant matter and soil are major risk factors for fungal keratitis), widespread use of traditional eye remedies that delay medical care, and limited access to specialist ophthalmology outside Dhaka and major cities.


What Causes Corneal Ulcers?

Understanding the cause is essential — because the treatment differs significantly depending on the causative organism.

Bacterial Keratitis — Most Common
The most frequent cause in Bangladesh, particularly following eye injuries from plant material, dust, or contaminated water. Bacterial keratitis is the most common cause of infectious corneal ulcers and should be the top differential diagnosis in patients presenting acutely with corneal infiltrates. It is often rapid in onset and progression, with potential for corneal melting and perforation within days. Dr. Agarwals

Common organisms include Pseudomonas aeruginosa (particularly aggressive — can destroy the entire cornea within days), Staphylococcus aureus, and Streptococcus pneumoniae.

Fungal Keratitis
In Bangladesh, fungal keratitis is the second most common cause — and arguably more difficult to treat. It typically follows trauma with vegetable matter: a paddy stalk, tree branch, or soil particle. Fungal ulcers tend to have feathery borders, satellite lesions, and a dry-looking surface — quite different from bacterial ulcers. They do not respond to antibiotics and require specific antifungal treatment, which many primary-level facilities do not stock.

Viral Keratitis — Herpes Simplex
Herpes simplex virus (HSV) is a significant cause of recurrent corneal ulceration, particularly the classic dendritic (branching) ulcer visible on fluorescein staining. It requires antiviral treatment — not antibiotics — and misdiagnosis with inappropriate steroid use can be catastrophic, causing rapid corneal destruction.

Acanthamoeba Keratitis
Less common but devastating. Associated with contact lens use, particularly in users who swim with lenses in or use tap water for lens cleaning. Acanthamoeba keratitis is extremely painful — often disproportionate to clinical signs — and resistant to most standard treatments. It requires prolonged specific therapy and is frequently misdiagnosed.

Risk Factors in Bangladesh

  • Agricultural work — plant matter injuries are the single biggest risk factor for fungal keratitis
  • Contact lens use — particularly poor hygiene or tap water use
  • Traditional eye remedies — a major cause of delayed presentation and worsened outcomes
  • Diabetes — impairs corneal healing and immune response
  • Previous eye surgery or chronic eye disease
  • Malnutrition and Vitamin A deficiency — particularly in children

Warning Signs — When to Seek Urgent Care

This is the most important section for patients and their families. A corneal ulcer is an ophthalmic emergency. If you or a family member has any of the following symptoms — especially after an eye injury — do not wait, do not apply home remedies, and do not use steroid eye drops without a doctor’s prescription.

The common symptoms of corneal ulceration include redness of the eye, circumcorneal congestion, pain, blurring of vision, photophobia (sensitivity to light), and watering of the eye — all with varying degrees of severity. Drashrafulhuq

Go to an eye specialist immediately if you have:

  • A white or grey spot on the cornea (the coloured part of the eye)
  • Sudden pain in the eye, especially after an injury
  • Significant sensitivity to light
  • Rapidly worsening vision
  • Discharge from the eye combined with corneal haziness
  • A red, painful eye that is not improving after 24–48 hours of antibiotic drops

A corneal ulcer that is caught early and treated correctly has an excellent prognosis. The same ulcer treated late or incorrectly can end in a blind eye. Time matters enormously.”

WHO Guidelines — Management at Each Level of Care

The WHO Regional Office for South-East Asia published guidelines for the management of corneal ulcers at primary, secondary, and tertiary care health facilities specifically for the South-East Asia region — which includes Bangladesh. Here is how those guidelines translate into practice: Alcon

Primary Level — First Contact Care

At the primary level — a community clinic, health centre, or general practitioner — the most important role is rapid recognition and immediate referral. A suppurative corneal ulcer is an ophthalmic emergency which should be referred to the nearest eye centre for proper management. Drashrafulhuq

Before referral, the primary provider should:

  • Apply broad-spectrum antibiotic eye drops (e.g. ciprofloxacin 0.3% or moxifloxacin 0.5%) hourly
  • Instruct the patient to continue drops frequently during transit to the referral centre
  • Never apply steroid eye drops — this is absolutely contraindicated without specialist assessment
  • Never pad or patch the eye tightly — this creates a warm, moist environment that accelerates bacterial growth
  • Document the history of injury, contact lens use, and any prior treatment

What primary care should NOT do:

  • Prescribe steroid eye drops — can turn a treatable ulcer into a perforating one
  • Apply traditional eye remedies of any kind
  • Delay referral to see if the condition improves over several days
  • Prescribe oral antibiotics as the sole treatment for a corneal ulcer

Secondary Level — District Eye Hospital

At the secondary level, the ophthalmologist should perform a full slit-lamp examination and attempt to establish the causative organism through corneal scraping and microbiological culture where possible.

Current guidelines recommend immediate treatment with broad-spectrum topical antibiotics as the first-line approach, with emphasis on empiric therapy for small noncentral ulcers. For bacterial ulcers, fluoroquinolones such as moxifloxacin 0.5% or ciprofloxacin 0.3% should be administered every 15–30 minutes for the first few hours, then hourly while awake for 24–48 hours, followed by gradual tapering based on clinical response. Key management principles at secondary level:

  • Corneal scraping for Gram stain, KOH preparation (for fungi), and culture sensitivity
  • Topical fluoroquinolone monotherapy as first-line for most bacterial ulcers
  • Fortified antibiotic drops (tobramycin + cefazolin) for severe cases
  • Natamycin 5% eye drops for suspected fungal keratitis — not fluoroquinolones
  • Acyclovir ointment for suspected HSV dendritic ulcers — not antibiotics
  • Cycloplegic drops (atropine) to reduce pain from ciliary spasm and prevent posterior synechiae
  • Corticosteroids may be considered after 48 hours of antibiotic therapy when the causative organism is identified and the infection has responded to therapy — but should be avoided in cases of suspected Acanthamoeba, Nocardia, or fungal infection. American Academy of Ophthalmology

Tertiary Level — Specialist Cornea Centre

At the tertiary level — a specialist cornea unit such as at Bangladesh Eye Hospital — management becomes more complex, particularly for cases that have not responded to initial treatment, have unusual organisms, or present with advanced corneal destruction.

A corneal ulcer is not a minor eye infection. It is an open wound on the most critical optical surface of the eye. Without the right treatment, started urgently, it can destroy the cornea within days and leave a patient permanently blind in that eye.

What is a Corneal Ulcer?

The cornea is the clear dome at the front of the eye through which you see. It has five layers, and its transparency is critical to sharp vision. A corneal ulcer — also called infectious keratitis — is a break in the corneal epithelium (the outermost layer) that extends into deeper layers, caused by microbial infection.

Unlike a superficial scratch, which heals in 24–72 hours, a corneal ulcer involves active infection eating into corneal tissue. If untreated, it progresses from the outer layers inward, can perforate the eye entirely, and allows infection to enter the interior of the eye — a condition called endophthalmitis, which can be blinding or require removal of the eye.

What Causes Corneal Ulcers?

Bacterial Keratitis

The most common cause in Bangladesh. Follows eye injury — even a minor scratch from a plant, fingernail, or piece of dust. The most aggressive organisms are Pseudomonas aeruginosa (can destroy the entire cornea in 48 hours) and Staphylococcus aureus. Onset is rapid — redness, pain, and a white spot developing within hours to days.

Fungal Keratitis

The second most common cause in Bangladesh — and the one I see most often in patients referred from rural areas. Typically follows trauma with vegetable matter: a paddy stalk, tree branch, or soil. Fungal ulcers are slower to develop, have characteristic feathery edges and satellite lesions, and do not respond to antibiotic eye drops. Natamycin antifungal drops are required — which many primary-level facilities do not stock.

Herpes Simplex Viral Keratitis

Caused by HSV-1 — the same virus as cold sores. Produces a classic dendritic (branching) ulcer visible on fluorescein staining. Requires antiviral treatment (acyclovir). The critical danger: if steroids are applied to an HSV ulcer — a common mistake — the infection spreads rapidly and causes geographic ulceration, potentially destroying the entire corneal surface.

Acanthamoeba Keratitis

Linked to contact lens use — particularly swimming with lenses, using tap water to clean lenses, or poor lens hygiene. Excruciatingly painful — often out of proportion to what is visible on examination. Resistant to most standard treatments and frequently misdiagnosed as HSV or bacterial keratitis. Requires specific prolonged therapy with polyhexamethylene biguanide (PHMB) drops.

Risk Factors Specific to Bangladesh

  • Agricultural work with plant material
  • Traditional eye remedies — the single most damaging factor I see in referred patients
  • Contact lens use with poor hygiene
  • Diabetes — impairs corneal healing and immune response
  • Vitamin A deficiency — particularly in malnourished children
  • Delayed presentation due to cost, distance, or minimisation of symptoms

Warning Signs — Seek Urgent Eye Care Immediately

Do not wait if you or a family member has:

  • A white or grey spot on the coloured part of the eye
  • Eye pain — especially after an injury or foreign body sensation
  • Rapid worsening of vision
  • Significant sensitivity to bright light
  • A red, watering eye that is not improving after 24 hours
  • Any eye injury involving plant material, soil, or a foreign body

Never apply these before seeing a doctor:

  • Steroid eye drops — accelerates infection and can perforate the cornea
  • Traditional remedies — breast milk, plant extracts, kohl, any non-prescribed substance
  • Antibiotic drops alone for a fungal or viral ulcer — delays the right treatment

Diagnosis — What I Do at the First Visit

A thorough assessment is essential before any treatment is started. At BEH, my assessment includes:

  • Slit-lamp examination — detailed visualisation of the ulcer’s size, depth, location, and characteristics
  • Fluorescein staining — reveals the ulcer’s exact shape; a dendritic pattern indicates HSV
  • Corneal scraping — material from the ulcer base is sent for Gram stain, KOH preparation (for fungi), and culture sensitivity
  • Anterior segment OCT — assesses depth of stromal involvement without contact
  • Intraocular pressure — elevated IOP can complicate management
  • Full history — mechanism of injury, contact lens use, prior treatment, duration of symptoms, and — critically — whether any traditional remedies or steroid drops have been used

The culture result takes 48–72 hours, but treatment begins immediately based on clinical appearance while awaiting results.

Treatment — How Corneal Ulcers Are Managed

Bacterial Ulcer

Intensive topical fluoroquinolone antibiotics — moxifloxacin 0.5% or ciprofloxacin 0.3% — every 15–30 minutes for the first few hours, then hourly while awake. For severe or central ulcers: fortified antibiotic drops (tobramycin + cefazolin). Cycloplegic drops (atropine) reduce pain from ciliary spasm. Patching is not recommended — it creates a warm, moist environment ideal for bacterial growth.

Fungal Ulcer

Natamycin 5% eye drops — every hour initially. Voriconazole drops or oral voriconazole for severe cases or when natamycin fails. Antifungal treatment continues for weeks — fungal ulcers are slow to resolve and require prolonged therapy and close follow-up.

Herpes Simplex Ulcer

Topical acyclovir ointment 5 times daily. Oral acyclovir for severe or recurrent cases. Steroids are absolutely contraindicated in active HSV epithelial disease. Only once the epithelium has fully healed, under specialist supervision, may carefully titrated steroids be considered for stromal disease.

Acanthamoeba Keratitis

Polyhexamethylene biguanide (PHMB) 0.02% drops — initially every hour, round the clock for the first 48 hours, then tapered over weeks to months. Treatment typically continues for 6–12 months. Pain management is a major component of care — these patients are often in significant distress.

When Surgery is Needed

Patients referred late — with deep, central, or perforating ulcers — may require:

  • Tissue adhesive (cyanoacrylate glue) for small perforations with bandage contact lens
  • Therapeutic penetrating keratoplasty — emergency corneal transplant to save the eye in cases of impending or actual perforation
  • Optical keratoplasty — elective corneal transplant after healing to restore vision lost to central scarring
